Diabetes Tool Kit

The tool kit includes materials designed to assist providers in caring for patients who have diabetes and to help people with diabetes manage their care:

  • Common Guidelines for Diabetes - All health plans participating in the coalition have agreed to endorse a common Guideline for Adult Diabetes Care, which lays out minimum expectations for care and highlights basic medical care for people with diabetes.
  • Eye Exam Report - This form was developed to facilitate communication between eye care professionals and primary care physicians, to inform physicians about the outcome of diabetes patients’ dilated eye exams.
  • Individual Patient Flow Sheet - The flow sheet, reflecting the recommendations found in the Guidelines for Adult Diabetes Care, will allow providers to track diabetes management over time by documenting diabetes medications, exams and test results.
  • Personal Diabetes Care Card - A wallet-size card that helps patients keep a record of their care.  The card has space for important phone numbers as well as spaces to record test values and regular exams.
  • “Manage Your Diabetes for a Healthy Life” patient brochure - A tri-fold brochure that provides current facts about the prevalence of diabetes and its complications. The brochure reminds patients what they should do to manage diabetes, helps patients to set diabetes care goals with their health care professional(s), and provides contact information for other resources.
  • “Manage Your Diabetes for a Healthy Life” wall poster - A 17X22 inch waiting room wall poster with information for patients on managing their diabetes.
