Family Health Plus is a state-sponsored program modeled on successful Child Health Plus. It is designed to make comprehensive health insurance available at no cost to lower-income, uninsured adults who do not have employer-sponsored coverage and are not eligible for Medicaid or Medicare. Family Health Plus enrollees receive health care services through participating health plans in their communities.

To qualify for coverage, an individual must be an uninsured resident of New York State between the ages of 18 and 65 who is not eligible for other public health insurance programs. In addition, the individual’s income must be under a specific amount that varies depending on the applicant’s family size and whether he/she lives with a child.
Family Health Plus coverage is comprehensive, with benefits that mirror those provided under the Child Health Plus program, including:

• Physician and nurse practitioner services
• Inpatient hospital services
• Prescription drugs
• Laboratory tests
• Diagnostic x-rays
• Durable medical equipment
• Radiation therapy, chemotherapy, and hemodialysis
• Emergency room services
• Inpatient and outpatient mental health, alcohol, and substance abuse services
• Emergency medical services provided by an ambulance service
• Emergency, preventive, and routine dental care (except orthodontia and cosmetic surgery)
• Emergency, preventive, and routine vision care
• Speech and hearing services
• Diabetic supplies and equipment
• Early and periodic screening, diagnostic and treatment services for enrollees 19-21 years old

People interested in Family Health Plus may apply for the program at convenient local sites that may include hospitals, community-based groups, and health care plans. Once enrolled in Family Health Plus, individuals will be able to choose among health plans participating in the program.
